This offer is good for purchases made from 3/1/04 to 3/1/12. You must submit the following materials within 30 days of receipt of your order to successfully claim your mail-in rebate: 1. Completed Rebate Form (click to download) . Rebate form must be completed in its entirety, and all materials must be legibly printed in ink or typed. The rebate form must be signed. 2. Copy of Your Receipt . This is the receipt that was emailed to you on the date of your order and will show your name, total amount, and billing/shipping address. Please print the receipt out and circle the amount paid and the date of purchase. 3. Packing Slip . Upon receipt of your packages of letters, you must remove the packing slip from the top of each box received and include all originals. No photocopies will be accepted. If you do not include the packing slip from all boxes, your rebate will be rejected. Claims postmarked or received after the earlier of the expiration date or 30 days from the date of receipt of order or incomplete or illegible submissions will be returned as ineligible. Please allow 8-12 weeks for processing of your rebate check. The Doctor Job will not provide a rebate if all of the terms and conditions have not been met. The Doctor Job is not responsible for any claims lost, damaged, or delayed in transit. Rebate checks are void if not cashed within 90 days of issuance and cannot be reissued.
